Wisconsin Sauk County 2 Close Medical Home Care Program

The Sauk County Board has voted to close the medical home care program.  Medical Care in home requires a physician salary, labor intensive process of filing Medicare claims, and stagnant reimbursement rates.  The Sauk County Board describes these as an ever expanding gap between revenue and expenses.  The findings suggest the program is unsustainable. 

Medical home care has a specific residence in the medical care continuum.  The medical care continuum is presented as below-

Hospital - works with Health Insurance Plans and Medicare

Nursing Home - works with Private Payment, Health Insurance Plans, and Medicare

Medical Home Care- Works with Private Payment, Health Insurance Plans, and Medicare

Non- Medical Long Term Care - Works with Private Payment, Public Funded Programs, Long Term Care Insurance Plans

Medical Home Care bills Medicare for the service provided in the home for the consumer under the orders of a primary medical doctor.  Medical Home Care offers the consumer access to Skilled Nursing Care, Physical Therapy, Occupational Therapy, and Certified Nursing Assistant in their own home.  Not everyone who needs the help of another will qualify for care covered under a Medicare plan. 

Non- Medical Care cannot bill Medicare for services provided.  Non- medical care offers the consumer access to Registered Nurse Care Planning, Senior Support Partners, Pet Visits, Assistance With Down Sizing Home Contents, Credit Repair Service. 

Medical Home Care has high cost to cover delivery of skilled services in a short defined period of time to help consumers get through an acute illness. 

Non- Medical Long Term Care is a less expense service delivered over longer periods of time to help the consumer with chronic conditions that require the help of another.

Wisconsin is primarily a rural setting which can be a challenge for County Planners who need to stay within a defined budget.  The loss of provision and changes with skilled medical care delivery can leave families trying to coordinate service on their own.
