Expect New Time Lines Wisconsin Family Care IRIS 2.0

Wisconsin is participating in a huge community level health care transition.  Implementation of the Wisconsin 2015 Budget changes are well under way.  The 2015 Wisconsin ACT 55 requires the Department of Health Services to make the Family Care Program available State wide by January 1, 2017.

The role of (ADRC's) County Aging and Disability Resource Centers and (MCO's) Managed Care Organizations now includes accurately informing program beneficiaries where they are within this change.  This is typically communicated in a timeline fashion.

When are the (MCO's) Managed Care Organizations closed and the (IHA's) Integrated Health (Insurance Plans) Agencies set to take over administration of the entire populations benefit.  This is expected to occur prior to Family Care becoming available State wide.

Home Care Path encourages the community to expect (delays) multiple announcements explaining a need to reconfigure the existing time lines .  This is a predictable aspect to any change of this size.  This roll out transitioning over to Wisconsin Family Care IRIS 2.0 began the day of the law change.

The time between Family Care Programs like all health care transitions is seen as an increased period of risk for the population being served.  The time between hand off is perceived as disruption and will require increased resources.

Home Care Path is constructing a multisided platform designed to help match families with dementia needs to local support partners who can give the primary care giver a much needed break.

Take a first peek https://www.homecarepathdementiasupport.com/en/infos/about at the open structure designed to strengthen a families access to the help they need in real time.  Families can list their need and support partners can list their availability.  This concise application exists to save families precious time and resources they so desperately need to effectively care for their loved one.
